The Admission Process
Initial Inquiry
Call us or submit the inquiry form below. Our admissions coordinator will reach out within one business day to answer your questions and discuss your loved one's needs.
Tour & Assessment
Schedule an in-person or virtual tour. During your visit we conduct a preliminary care needs assessment to determine the appropriate care level and discuss availability.
Document Review
We review the required medical and legal documents (listed below) and work with your healthcare providers to ensure a smooth transition.
Care Plan Development
Our nursing team creates a personalized care plan tailored to your loved one's specific needs, preferences, and goals — with your input at every step.
Move-In Day
We coordinate a comfortable, stress-free move-in. Our team provides full orientation support and stays available for any questions from residents and family.
What to Prepare
Required Documents
- ✓Recent physician's physical exam (within 12 months)
- ✓Current medication list with dosages
- ✓Medical records and relevant specialist reports
- ✓Functional assessment / ADL evaluation
- ✓TB test results (within 6 months)
- ✓COVID-19 vaccination record
- ✓Government-issued ID
- ✓Medicare / Medicaid / insurance cards
- ✓Power of Attorney (healthcare and financial)
- ✓Advance Directive / Living Will (if applicable)
- ✓Signed admission agreement and residency contract
